Volunteer Fire Department Evaluations


Evaluation forms can now be submitted online via the submission forms below. It is best to use an updated version of Internet Explorer when completing the forms. 

The Funding Package Affidavit cannot be submitted online at this time. The form below must be completed by ALL FIRE DEPARTMENTS, be notarized and mailed to the mailing address at the bottom of the page.
